Embedded as QSHE lead across a multi-workstream capital expenditure programme at a large manufacturing and brewing site, coordinating safety supervisors across concurrent contractor crews. Scope covered RAMS review, permit-to-work governance, toolbox talk content and management-of-change documentation for electrical and mechanical works.

Delivered a site-specific Pre-Energisation Checklist aligned to the client's electrical safety standard, with arc flash PPE categories and formal signatory blocks built in — closing a gap between engineering handover and safe energisation.

Outcome

Standardised pre-energisation sign-off adopted as the default control for CapEx electrical handover across the workstream.

Audit scope and commercial proposal accepted as the basis for a phased compliance programme.

5-Why / RCPS SIF-P Determination

Led the investigation into a voltage stabiliser arcing incident on a production line, running a structured 5-Why and root cause problem-solving process. Applied a Serious Injury and Fatality Potential (SIF-P) determination methodology to classify actual and potential severity, and produced findings and corrective actions in the format required for stakeholder and leadership review.

Outcome

Formal RCA report and corrective action plan delivered within the incident's investigation window, with findings tracked to closure.
